Coach Accreditation

Please read all points below before ticking the boxes, as the points contain important information needed for a successful application.

Apply for accreditation.

To register your details and apply you will need:

• 2 x email addresses of references (NB references cannot be accepted from family or friends)
• Your contact details including full address and telephone number(s)
• DBS information, including disclosure number, completion date and renewal date. If taken by Dudley Council you will need to provide your DBS number and date of completion(NB it must be within the last 3 months at the time you meet the validator to activate your membership), if not taken with Dudley Council you must be part of the update service otherwise you will not be able to complete the registration process and will need to undertake a new DBS check with update service (see here for update service information - https://www.gov.uk/dbs-update-service). 3 yearly renewals are not required if the update service applies.
• Safeguarding certificate details, including name, completion date and renewal date
• Insurance details, including company name, completion date and renewal date
• Your coaching qualifications (will be selectable from a dropdown list) including completion dates and renewal dates (where applicable).

• When you attend the validation meeting please ensure you bring the following.

  • Photo ID (passport or driving licence)
  • DBS certificate
  • All national governing body certificates
  • 1st Aid certificate
  • Safeguarding certificate
  • Insurance certificate if you are operating as an independent deliverer not part of a larger company
• You have thoroughly read, understood and agree to adhere to the Guidance for Safer Working Practice (Click Here to read).

• Your data will be processed and stored in accordance with GDPR. Data stored on SiSS will be shared with Dudley educational establishments who have elected to subscribe to SiSS and will access the system securely. Your home address and home telephone number will not be visable to schools.
